For example, for acute inpatient treatment, the services must be designated as “active” and provided under an individualized treatment plan. These programs must also be supervised by a physician and expected to improve the condition of the patient.

To gain coverage for inpatient detox, the member must be diagnosed with Substance-Related Disorder, according to standards of the DSM-5. The withdrawal or detox treatment must include monitoring or intervention happening more frequently than hourly.

Similar criteria are outlined for admission to an inpatient rehabilitation facility as well.

Coverage is available for the treatment of both drug and alcohol abuse when it’s seen as a medical necessity, and for each specific element of rehab including detox, inpatient admission, and outpatient services, particular guidelines and criteria must be met in the determination of medical necessity.
